Aldosterone receptor antagonists, including spironolactone and eplerenone, have been shown to promote reverse remodeling and improve mortality in adults with chronic heart failure [43,44]. No studies specific to DCM have been reported. Despite significant clinical experience with these agents in pediatric cardiac patients for their potassium-sparing qualities, studies in pediatric heart failure are not available. Based on data from adults with chronic heart failure of any etiology, it is reasonable to consider aldosterone receptor antagonists in pediatric patients with systemic ventricular dysfunction (recommendation Class I, LOE C) [35].
